Your first step is to establish an account with NASR. Complete the Study Account Information Sheet and fax it to Nutrition Assessment Shared Resource (NASR) at (206) 667-7864. Be sure this form is complete especially contact information. Please call NASR to discuss time period information. The information on this form is entered into a tracking system that keeps your data secure and separate from other studies. This system is also used for billing.
You will receive a unique 3-letter project code specific to the processing of your study. If you have multiple studies, each one should have its own 3-letter code. If you begin a new study, it too will need its own unique code. Please fax us a separate account information form for each new study.

If a study chooses to use the Soy or Caffeine questionnaire, there is a $75 one-time user fee for the printing the questionnaire. (This fee does not include processing). You may print as many as you want, in black and white or color. No changes will be made without a custom fee.

Your study is responsible for QC. There will be a $10 charge for each "Other" food listed. This charge can be waived if your study sends brand name and isoflavone or caffeine information. It is very time consuming research to get the isoflavone/caffeine content (if even known).
When the questionnaires are completed, the originals or copies should be sent, along with the FFQ Questionnaire Cover Sheet to:

Each Soy Questionnaire is data-entered and processed at the cost of $3.00. The Caffeine Questionnaire data-entry and processing is $1.50 each. When the batch is completely entered and processed, your study will receive an email saying your data is available on the website. To access you will need a username and password that was set up when the information on the Account Information Sheet was entered into tracking. Please request this username and password from ffq@fhcrc.org.
The hard copy Questionnaires are held at NASR for 7 days post processing before being sent back to you. This is done in case there are any questions about entry or data.
